Adding a form to a menu in Joomla!

  1. Log in to the Joomla! back-end.
  2. Create and save your form
  3. Create a new Menu using the Menu Manager. (Accessable from the Top menu “Menus > Your Desired Menu (Probably Main Menu)”)
  4. In the Menu Item Manager, click the New toolbar button “On the upper right corner of the screen”
  5. From there follow the instructions in the following Images


Select "JForms"


Select "Standard Form Layout"


Select a title for you form and select the form to display


Select the form from the list


Click "Save"
